Clinic Policies
For All Patients
- Payment for visits to our clinic is by a valid provincial care card or
cash. All valid provincial medical cards are accepted except Quebec.
If your visit is for a service not covered by your care card, cash payment
is required ( you would be informed of this prior to the service).
- Picture ID may be requested for a first visit.
- We see walk-in patients in the order they come in, unless there is a
medical emergency or the patient only requires an injection.
- If you are in the office for an injection only, you will be put in the
next available room. Please inform the receptionist the visit is for
an injection only.
- Please inform the receptionist if more than 1 person will be consulting
the doctor so all appropriate charts are prepared.
- Please inform the receptionist of your reason for the visit, so she can put
you in the appropriate room and do any appropriate tests before the doctor
sees you.
- A visit is for a single problem. Please do not save up multiple
problems for a single visit. A pap smear and a breast exam constitute
an entire visit.
- All test results are checked by the doctor of the day. If you have an
abnormal result the doctor will call you to come in for follow-up unless you
have been adequately treated already. If your illness persists or
deteriorates, you should follow-up regardless of the lab result.
- For STD blood tests like HIV, you must follow-up to receive the results
2 weeks after you get the bloodwork.
- No medical or visit information, including tests results will be given
over the phone.
- Plan B ( the morning after pill) is available from the doctor for $15.
- Passports will not be done for walk-in patients.
Family Practice Patients
- You must have been accepted as a family practice patient by Dr.
Jacobsen, Dr. Ho, or Dr. Byczko to be considered a family practice patient.
- Once you have been accepted as a family practice patient by the doctor,
that doctor is your family doctor not the others.
- We will not generally take phone calls for medical advice. Please feel
free to visit the clinic anytime as a walk-in or book an appointment with
your doctor.

- If you have an urgent problem outside of office hours, advice is
available by calling our phone number.
- If you are taking blood thinners ( Coumadin/Warfarin), you must get
regular blood tests (INR) and have a phone number where we can reach you at
all times in case your level is not right. If the level is too low or
too high it can be potentially life-threatening.
- We do not renew prescriptions over the telephone. Please ensure
you have enough medication until your next visit or make an appointment to
review and renew your prescriptions. This enhances the quality of your
care and safety.
- A passport guarantee will only be done if you have been seeing the
doctor for greater than 2 years and have had multiple visits. The doctor has to know you
well enough to be comfortable with signing the guarantee.
Walk-In patients
- No appointments, walk-in only.
- No telephone prescription renewals.
- No renewals of potentially addictive medications such as narcotics and
benzodiazepines.
- No telephone advice.
- No on-call service.
- No passport guarantee.
