What to bring to the visit

1. Your valid carecard or cash if you do not have a valid carecard.
2. All your medications or a current list of all your medications and the dosages.
3. A list of all your medical conditions including allergies.
4. Any pertinent consult reports, lab or Xray results.
    

If you need a form completed

1. Please show the form to the receptionist to determine whether the doctor can complete the form for you and whether there is a cost.
2. If your work nor MSP covers the cost of the form, then you will be expected to pay.
3. Often you are told that a form merely requires a doctor to sign it, but in reality the doctor is signing that he/she finds you fit and this requires an examination.
4. Sick notes are not covered under MSP and therefore there is a fee for this.
